## Requesting a ride, in three steps

From the form filled in a few days ahead to the doorbell that morning: what the app does, then what your team does.

### A five-field form
  Pickup address, date and time of the appointment, destination address, whether a wheelchair is needed, phone number. Nothing about the illness or the reason for the visit: the trip doesn't need it. Each request reaches your team by email, and they can find it in the back office too.

### Your team calls back and confirms
  On the phone, they check what a field can't tell them: stairs with no elevator, who's coming along, a prescription if the patient's health system requires one. They set the pickup time, allowing for the drive and check-in at the facility, then confirm by phone or email.

3. 3

### The driver rings at the agreed time
  They help the patient out of the house and into the vehicle, a car or a wheelchair-accessible van, as agreed. A door code or a neighbor with a spare key, noted during the call, saves the driver from waiting outside.

The app never confirms a ride on its own, and that's deliberate: only your team can match the time of a treatment, the patient's mobility and the accessible vehicles available.

## What to have ready before the ride

### A photo ID

In your bag the night before: the front desk at the facility may ask for it, and the driver won't be able to turn back and fetch it.

### Your health system's paperwork

Your prescription or authorization, if your health system requires one for this ride: the team tells you when they call.

### Your medication list

Up to date, on paper, for the care team receiving you. It stays with you: neither the form nor the app asks for it.

### Wheelchair, cane or walker

Mention it in the request: it decides which vehicle comes, a car or an accessible van, and how much time to allow for getting you settled.

### A family member in the loop

They know when you leave and have the team's number: they're often the one who calls for the ride home, or who fills in the request for you.

### A one-time appointment

A 10:30 a.m. test at the hospital, 15 miles from home. The patient's daughter sends the request three days ahead; the team calls her back the next day, sets pickup for 9:15 and notes that her father walks with a cane.

### Three sessions a week

Dialysis or physical therapy, Monday, Wednesday and Friday, for months. This routine is set up once with the team, which plans every trip there and back: the patient doesn't request rides one by one. In the app, a page explains how to report a rescheduled session.

### Discharge from the hospital

Discharge time depends on the doctor's rounds and the unit's paperwork: it's rarely known the day before. If you leave the time field optional, the request goes out without it; on the day, the family member calls as soon as the patient is ready, with the number one tap away in the app.

## Medical transportation app: questions from transport teams

**### Can patients book their ride directly in the app?**

No: they send a request, and your team confirms it by phone or email. A patient's ride depends on the time of treatment, their mobility and which accessible vehicles are free that day; no slot calendar can weigh all that. So the app shows no times to tap, and nobody assumes their ride is set before you call.

**### Can a family member make the request?**

Yes. Add a choice at the top of the form, "Who is making this request?" (the patient, a family member, a caregiver), and a field for the number of the person to call back. A son arranging his mother's ride from another city enters her address and his own phone number: he's the one the team calls.

**### How do we handle rides for sessions three times a week?**

With a single call, not a request for every session. The patient, a relative or the treatment center gives you the days, the start time and the usual length of the session, so the ride home is planned. After that, the app only matters when something changes: a page in it says who to notify and how far ahead.

**### How do patients call for the ride home?**

They call the team on the way out: from the app's contact page, the number dials with one tap. The end of an appointment can't be predicted to the minute, which is why the ride home starts with a phone call rather than a time fixed in advance. State in the app how long the vehicle usually takes to arrive after the call.

**### What medical information should we ask for?**

None. Ask in the app only for what the trip needs (address, time, destination, wheelchair, phone number). No reason for the visit, no diagnosis, no prescription attached. Even kept that short, a request reveals that someone is going for care: check how the GDPR, if you operate in Europe, and your own country's rules apply before you put the form online.

**### Can we alert all patients about a snow day?**

Yes. Sent right away or scheduled the evening before, a push notification reaches every user, for example "Snow expected tomorrow: our vehicles will leave earlier, and the team will call you tonight." A strike or a closed road can be announced the same way. Only people who have the app and allow its notifications get it; the team calls the other patients.

**### Can the app replace my dispatch software?**

No. It doesn't plan routes, assign vehicles, track their location or handle billing: your dispatch software keeps that job. The app sits upstream, where patients request a ride, get ready for it and find your number. Requests received through the form export as CSV if you want to bring them into that software.
